Targeted Case Management

Targeted Case Management is a Title 19-funded service in which a Case Manager provides assessment, referral, coordination and monitoring of support services for individuals with disabilities to assist them meet their needs and achieve their desired goals.

The following individuals may be eligible:

  • Children who have applied and are eligible for the Intellectual Disabilities Waiver through DHS.
  • Adults 18 and over who have Title XIX and have a diagnosis of Intellectual Disability, Brain Injury, or Developmental Disability, and demonstrate a need for services.

  • Individuals may complete an application at the Community Services Office.  Individuals with an intellectual disability or brain injury may also complete an application at the DHS Office (1413 1st St W, Independence, IA 50644).
